SOHOHost Web Form Creator
Use this form to create your own web forms for your website. You can create
Feedback Forms, Guest Books, Order Forms and Mailto Forms.

Instructions
- Fill in Form Options Section
- Type in Form Name.
- Type in Form Action. If the information from this form is to be
e-mailed back to, type in "mailto:your e-mail address. If you are
using a cgi script on another server, put it's full URL here.
- Type in Method (Usually POST).
- Leave Target as is.
- Leave Enctype as is.
- Click Start Form Button
- Fill in Form Field Section for each form field you want to add.
After each field, click the Add Field Button. When you are done adding
fields, click the End Form Button and you will see your form generated
at the bottom. You can also preview your form, by clicking the Preview
Button. Copy and paste into your web page editor.
- Choose the type of field.
- If you want a default value, type it here.
- Type in the name of the field (i.e. name, address, city).
- Type in the size of the field (i.e. 20, 30, 40).
- Type in the maximum length of the field (usually the same or
higher than the size of the field above).
- Type in the number of rows (for Text Area fields only).
- Type in the number of columns (For Text Area fields only,
usually the same amount as size of field above).
- Check Add Tag After Form Item
- If your field is a checkbox or radio button, check the "Is
the checkbox/radio button checked", if you want it checked as
a default.
